EMPLOYEE INSURANCE BENEFITS
Health & Welfare Benefits are available to employees working 20 or more contract hours per week:
- Blue Shield of California or Blue Cross of California
- Ameritas Dental
- VSP (vision)
- Standard Insurance Company (life insurance)
The applicable collective bargaining agreement determines every employee's share of premium costs
Participation in benefits programs is mandatory for employees who work at least 0.90 FTE.

DENTAL INSURANCE - Ameritas Dental Coverage
Member account registration after your benefit effective date:
1. Go to ameritas.com/sign-in and select Member Sign In
under Dental, Vision & Hearing.
2. Choose your account type, validate your identity, and
follow the prompts to create your account.
VISION INSURANCE - VSP Vision

Coverage begins on the first day of the month following 30 days of employment. If you elect not to enroll at the time of initial employment, you may enroll during the open enrollment period.
LIFE INSURANCE (Provided by FBUSD)
All eligible staff members are offered life insurance as follows:
Eligible certificated staff (those working 50% or more) are insured for $20,000 (base). Your insurer is Standard Insurance Company, and coverage begins on the first of the month following enrollment/employment.

Action Required: When filling a prescription, please ensure the pharmacy enters the first three letters of your account ID number from your Blue Shield/SISC card. Note: If the pharmacist omits these first three letters, they will not be able to find your account in their system.

Fort Bragg Unified School District offers its employees the opportunity to purchase supplemental benefits through American Fidelity. Supplemental benefits are specialized insurance policies that employees may purchase voluntarily. These optional insurance policies do not take away from your regular medical, dental, vision, and life coverages through the district. Instead, they are simply other insurances you may purchase at your option that go above and beyond the coverage already provided under your regular district insurance.
If you choose to sign up for any supplemental coverage through American Fidelity, your premiums may be paid through payroll deduction for convenience.
