Staywell Health Screening Opportunity
Good morning! Your Staywell Board of Directors has adopted a Health Screening Program for the 2011-12 year, to re-emphasize our mission to STAY WELL. The Health Screening program involves a health screening event (biometric screening). We have contracted with Summit Health for the Biometric Screening services. If you participate in the biometric screening, and have a preventive visit with your physician, you will receive a $100 gift card. The attached newsletter and 2011 Staywell Biometric Screening FAQ, should answer any question you may have. A copy of the newsletter will also be included with your September paychecks.

The Frequently Asked Questions has all the info you’ll need, but I should emphasize ENROLLEES and SPOUSES/DOMESTIC PARTNERS (if they are covered by Staywell) are eligible to participate in the Health Screening Program and to receive a $100 gift card.
The biometric screening day is October 26, 2011, 7-11 a.m. at the John Diederich Center. We are asking everyone to sign up on-line. It will take 15 minutes and you will receive your results and have an opportunity to discuss you results with your screening nurse. Here’s the link:

If you are unable to participate that day, you may order a home test kit and complete the screening that way. You will miss being able to discuss your results that day, but you will still be able to take your results and discuss them with your doctor at your preventive appointment.
